BEFORE A BAPTISM CAN BE SCHEDULED, PLEASE ENSURE THAT YOU HAVE BEEN REGISTERED FOR AT LEAST 90 DAYS, YOU SUBMIT THE BAPTISM REGISTRATION FORM AND THAT YOU HAVE OBTAINED AND SUBMITTED THE REQUIRED DOCUMENTATION.

IF YOU ARE NOT A REGISTERED PARISHIONER, YOU ARE WELCOME TO HAVE YOUR CHILD BAPTISED AT OUR PARISH. YOU MUST ALSO SUBMIT A COMPLETED BAPTISM REGISTRATION FORM, OBTAIN AND SUBMIT THE REQUIRED DOCUMENTATION, PAY A $100 NON-PARISHIONER FEE, AS WELL AS SUBMIT A SIGNED AND SEALED LETTER OF PERMISSION TO BAPTISE OUTSIDE OF YOUR PARISH.


GODPARENT REQUIREMENTS

  • Canon 872

    The godparent’s role with the parents is to present the infant for the sacrament of Baptism and help the child live a life befitting a baptized Catholic Christian.
  • Canon 873

    There may be only one male godparent or one female godparent or one of each.
  • Canon 874

    1. Be designated by the parent and must be mature enough to undertake this responsibility;

    2. Be at least 16 years of age;

    3. Have received the three sacraments of initiation namely Baptism, Eucharist and Confirmation and
    live a life of faith in keeping with the function to be taken on;

    4. Not be bound by any canonical penalty legitimately imposed or declared by the Church;

    5. Must not be the father or mother of the one to be baptized;
  • Canon 874

    A baptized person who belongs to a non-Catholic Christian community may not be admitted as a godparent but as a witness to the Baptism together with a Catholic godparent
  • IF THEY ARE MARRIED
    Canon 874

    Valid wedding in the Catholic Church

    Catholics in an invalid marriage (not according to Catholic Church regulations) and those that are cohabitating (living together without marriage) cannot serve as a sponsor.
  • IF THEY ARE REGISTERED PARISHIONERS OF ANOTHER PARISH
    Canon 874

    They will need a letter of eligibility from their home parish Pastor

IN SUMMARY WE NEED THE FOLLOWING REQUIRED DOCUMENTATION:

  • Documents needed from parents:
    1. Baptism Registration Form (Scroll to bottom of page for link to form).
    2. Birth certificate or hospital verification such as a postpartum discharge letter (MUST have both baby’s name and parents’ name on it.) This can be submitted after the registration form but we will need it before scheduling a baptism.
    3. FOR NON-PARISHIONERS ONLY: A signed and sealed (parish seal) letter of permission to baptize outside of YOUR parish.
  • Documents needed from each Godparent:
    1. NON-PARISHIONER GODPARENTS: A letter of Good Standing from home parish with signature and parish seal
    2. A reissued Baptismal Certificate that has notations indicating you have completed your other sacraments. This Baptismal certificate must have been reissued within the year. Notations should include First Communion, Confirmation, and Marriage information. If the parish of your baptism does not have these notations, then certificates are needed for First Communion, Confirmation, and Marriage (if applicable) and you must reach out to each parish to request them.

ANTES DE QUE SE PUEDA PROGRAMAR UN BAUTISMO, POR FAVOR ASEGÚRESE DE HABER ESTADO REGISTRADO(A) CON NUESTRA PARROQUIA DURANTE AL MENOS 90 DÍAS, DE ENVIAR EL FORMULARIO DE REGISTRO DE BAUTISMO Y DE HABER OBTENIDO Y ENVIADO LA DOCUMENTACIÓN REQUERIDA.


SI USTED NO ES UN FELIGRÉS REGISTRADO, PUEDE BAUTIZAR A SU HIJO EN NUESTRA PARROQUIA. TAMBIÉN DEBE PRESENTAR UN FORMULARIO DE REGISTRO DE BAUTISMO COMPLETADO, OBTENER Y PRESENTAR LA DOCUMENTACIÓN REQUERIDA, PAGAR UNA CUOTA DE $ 100, ASÍ COMO PRESENTAR UNA CARTA FIRMADA Y SELLADA DE PERMISO PARA BAUTIZAR FUERA DE SU PARROQUIA.



REQUISITOS DE PADRINOS

  • Código de Derecho Canónico 872

    El papel del padrino con los padres es presentar al infante para el sacramento del Bautismo y ayudar al niño a vivir una vida digna de un cristiano católico bautizado.
  • Código de Derecho Canónico 873

    Puede haber solo un padrino o solo una madrina o uno de cada uno.
  • Código de Derecho Canónico 874

    1. Ser designado por el padre y debe tener la madurez suficiente para asumir esta responsabilidad;

    2. Tener por lo menos 16 años de edad;

    3. Haber recibido los tres sacramentos de iniciación, que son, el Bautismo, la Eucaristía y la Confirmación y
    vivir una vida de fe acorde con la función a asumir;

    4. No estar obligado por ninguna pena canónica legítimamente impuesta o declarada por la Iglesia;

    5. No debe ser padre o madre del que va a ser bautizado;

    Un bautizado que pertenezca a una comunidad cristiana no católica no podrá ser admitido como padrino sino como testigo del Bautismo junto con un padrino católico
  • SI ESTÁN CASADOS

    Boda válida en la Iglesia Católica

    Los católicos en un matrimonio inválido (no de acuerdo con las normas de la Iglesia Católica) y los que están cohabitando (viviendo juntos sin matrimonio) no pueden servir como padrino.
  • SI SON feligreses registrados en otra parroquia

    Necesitarán una carta de elegibilidad del párroco de su parroquia de origen.

EN RESUMEN NECESITAMOS LA SIGUIENTE DOCUMENTACIÓN REQUERIDA:

  • Documentos necesarios de los padres:
    1. Formulario de registro de bautismo (Desplácese hasta la parte inferior de la página para ver el enlace al formulario).
    2. Certificado de nacimiento o verificación hospitalaria como una carta de alta posparto (DEBE tener tanto el nombre del bebé como el de los padres.) Esto se puede presentar después del formulario de inscripción, pero lo necesitaremos antes de programar un bautismo.
    3. SÓLO PARA NO FELIGRESES: Una carta firmada y sellada (sello de la parroquia) de permiso para bautizar fuera de SU parroquia.
  • 1. PADRINOS NO FELIGRESES: Una carta de buena reputación de la parroquia de origen con firma y sello parroquial.
    2. 2. Un Certificado de Bautismo reexpedido que tenga anotaciones indicando que usted ha completado sus otros sacramentos. Este Certificado de Bautismo debe haber sido reexpedido dentro del año. Las anotaciones deben incluir información sobre la Primera Comunión, la Confirmación y el Matrimonio. Si la parroquia de su bautismo no tiene estas anotaciones, entonces los certificados son necesarios para la Primera Comunión, Confirmación y Matrimonio (si es aplicable) y usted debe ponerse en contacto con cada parroquia para solicitarlos.

Please complete the Baptism Registration Form (click on the link/blue button above) and have the soon to be Godparent(s) obtain their sacramental records. Please send all the original documents to the parish office. Email the documents to: office@phoenixlatinmass.org or mail/drop off the documents at:

We cannot schedule a baptism until we have all the necessary documents. Keep in mind that it could take one to a few months to get your baptism scheduled depending on the documentation needed. We ask that expecting mothers to register at least two months before their baby is due.
The baptism will typically be scheduled for a Saturday morning unless otherwise arranged.

No podemos programar un bautismo hasta que tengamos todos los documentos necesarios. Tenga en cuenta que podría tomar de uno a algunos meses programar su bautismo dependiendo de la documentación necesaria.
